You can normally get a National Entitlement Card application (and NEC 003 form) from:
- all Midlothian libraries
- main council offices
- Midlothian Community Hospital
- Pathhead and Penicuik Health Centres
- the Rosewell Development Trust (next to Rosewell Primary School on Carnethie Street).
Which forms to fill in
If you are registered disabled, please complete the National Entitlement Card application form NEC 002.
Mental health issues
If you have mental health issues, please also submit form NEC 003. Your mental health practitioner must complete it.
Epilepsy
If you are 17 or over and have epilepsy (which means you cannot get a driving licence) you must also submit form NEC 003. It must be signed by your GP, Epilepsy Specialist Nurse or Neurologist.
Learning disability
If you have a learning disability, please also submit form NEC 003. It must be signed by someone like a Support Worker or SEN teacher.
What to submit with the form
You will need to provide a colour passport-style photograph and proof of your eligibility and address.
These documents are acceptable as proof of age, but are only needed if you are 11 to 26 years old:
- Birth Certificate
- Passport
- Photo Driving Licence
- Adoption Certificate
- Marriage Certificate
- Young Scot or other PASS hologrammed card
- Medical Card
- Residence Permit issued by the Home Office to EU Nationals
- HM Forces ID Card
- Home Office Letter IS KOS EX or KOS EX2
These can be used for proof of address:
- Council Tax bill issued in the current financial year Apr-Mar
- Current UK Driving Licence
- Gas, electricity or landline phone utility bill dated within the past 3 months
- Bank, building society or credit union statement or passbook dated within the past 3 months
- Recent original mortgage statement
- Current valid television licence
- Child Benefit award letter
- Current DWP Benefit letter
- Letter from a care or residential home confirming that you are resident
- Letter from Midlothian Council confirming that you are a resident in the Council area
- Letter from a school, college or university confirming your home address
- Letter from the Home Office or Immigration Office confirming your home address
- Confirmation from an electoral register search
- Record of home visit by council or Government official
- Letter from HMRC confirming your home address
- Court order quoting your home address
- Authorized NEC 003 Mental Health certificate
- Authorized NEC 003 Epilepsy certificate
- Authorized NEC 003 Learning Disability certificate
You can use these documents as proof of eligibility:
- Disabled Blue Badge issued in Scotland
- DVLA V11 certificate showing "disabled taxation class" or DLA 404
- DBD 384 Disability Living Allowance (DLA) higher or middle rate care component
- DBD 384 Disability Living Allowance (DLA) higher rate mobility component
- Standard or enhanced rate of the mobility component of the Personal Independence Payment (PIP)
- Standard or enhanced rate of the daily living component of the Personal Independence Payment (PIP)
- Registered Blind certificate from Lothians RNIB in Edinburgh
- Current Veterans Agency award letter confirming receipt of mobility supplement
- Attendance Allowance award letter
- War Pensions Constant Attendance Allowance
- Authorized NCT 002 Mental Health certificate
- Authorized NCT 003 Epilepsy certificate
- DVLA letters D206, D220, D235 or NS57 stating failure of eyesight standard to drive, or revoking of driving licence on medical grounds of physical fitness
- Audiologist written report, letter from the Society for the Deaf, or a letter from a Hospital Consultant
- Hospice admittance letter
- DS1500 report
- Written confirmation from prosthetics clinic/centre, physiotherapist or rehabilitation centre
- Letter from Hospital Consultant if applying on the grounds of suffering from a progressive degenerative disorder
These are proof of eligibility for a Companion Card:
- DBD 384 Disability Living Allowance (DLA) higher or middle rate care component
- Standard and enhanced rate of the daily living component of the Personal Independence Payment (PIP)
- Attendance Allowance award letter
- War Pensions Constant Attendance Allowance
- Registered Blind certificate from Lothians RNIB in Edinburgh
- Letter from a care or residential home or hospital, confirming that you are a resident, and that if you were in your own home you would be eligible for:
- the higher/middle rate of the care component of Disability Living Allowance
- or a Personal Independence Payment
- or Attendance Allowance.
Return the form
Take the completed form to any Midlothian library, when open, for verification and validation. Currently, only Dalkeith, Newtongrange and Penicuik libraries are open.

When you will receive the new card
It can take up to 14 working days from the form being checked at the library to the card arriving at your home.
