Midlothian's Young People Awards 2023: nomination form

Midlothian’s Young People Awards 2023 are kindly sponsored by:

The awards ceremony will be on Tuesday 28 November 2023 at Rosslyn Chapel. Nominees should be available to attend.

Before you start

1. Read about who is eligible for nomination.
2. Tell the young person that you're nominating them, and get their consent, or if they are under 13, their parent or guardian's consent. New data protection legislation requires this. Read the Education Services privacy notice.
3. Have some information ready to paste into the form. You only have 10 minutes to complete each page, after which it will time-out.
   
   We will ask you:
   • What sort of activity is the young person involved in?
   • How committed are they?
   • What do they do that makes them stand out as special?
   • What difference have they made and to whom?
