If the pupil has a disability or any additional needs, we will need documentary evidence. Please send photos/scans of the documents to pupil.placement@midlothian.gov.uk, within 14 days from now.
Complex needs
Do not use this form to request a place if your child has complex needs. Please discuss with the Primary School Head Teacher.
