Transition from Children's Services to Adult Services
The ASN Transition Forum
The ASN (Additional Support Needs) Transition Forum is an Education led meeting, held twice per year.
A young person (aged 14+/S3+) should be referred to the ASN forum if the team around the young person feels that significant support will be required to ensure a positive transition on from school to a positive future destination.
The Forum provides support and advice on the planning/resourcing of post school transition plans for all young people who are at stage 3 or 4 of the Staged Intervention Matrix (please ask school if you are unsure what stage your young person is).
During the Forum, the referrer will present information about the young person, the panel will discuss and provide advice, updates and suggest actions to be taken.
The Forum is used to track and monitor the transition planning process for young people with additional support needs throughout their transition.
Midlothian Council use information from the Forum to help plan for services required to meet the needs of the young people.
