ASN Transition Planning Meetings

Education follow guidance from the Midlothian 14+ ASN Transition Planning Process. There is a requirement to follow this process for all young people with an ASN receiving stage 3 or stage 4 support (ASN Matrix of Need) in education, with planning starting  from age 14 years (S3). They will be tracked, monitored and supported throughout their transition journey to ensure appropriate post school supports are in place for their school leaving date.

Education will organise a transition planning meeting for the young person at least 12 months before leaving school for young people at stage 3 or 4 on the Matrix of Need. The number of people involved, and the frequency of meetings will be reflective of need.
